How to report a motel to the health department?

How do you report a motel to the health department? I recently stayed at a place that was advertised online and looked so much better there than it actually was. The place was falling apart, had spiders, flies, the AC didn't work and worst of all it had bed bugs. The floor had a hole in it, the building literally moved under your feet and the shower to the wall caved in if you touched it. Definitely a health hazard especially with the bed bugs. So how can I report this place and possibly get my money back? I'd rather just report them and save another traveler from having to go through the same thing I did. Thanks!
